Term Life Insurance - Health Issue

Hello,

I am trying to obtain term life insurance and am looking for some helpful guidance from anyone that has had a similar issue in the past.
- Current coverage through employer of basic life 5X salary, AD&D 1X salary. Salary of ~$100k/year.
- I earn ~70% of household income.
- I have two young children to support for the next 18 years plus a $300k mortgage.
- Spouse has a $750k/20 year policy for $22/month with 20 years remaining.
- I recently applied for a $1M/20 year term policy and was rejected. I expect this was due to my health.
- No health issues to date, however BMI of 40%, 5'10, 275 lbs, 31 years old.I know I need a $1M policy to cover my family and am committed to getting into a "insurable" health range. I know the health questionnaire will ask for weight range over the past year and I will likely need to obtain this coverage over the next 18 months in a couple different policies. I'm not sure exactly what my target health is though and have a few additional questions:
1. Is there an accurate way to estimate annual costs for coverage amount based on an expected health/weight? For example, if I want a 20 year $300k policy, what health range would I need to come in at for this policy at $500/year?
2. If I obtain a small (~$250k) shorter policy now such as 10 years just for peace of mind, is that going to put me into a higher cost situation if I apply for a larger 20 year policy in the next 1-2 years?
3. If I am able to obtain a shorter policy now, how long should I maintain that after obtaining a longer policy at a lower cost?
4. For individuals with multiple term policies, is there any negative impact from an insurance provider such as higher costs?
